The Maine Hospital Association (MHA) has created a standard form that most people in Maine use to create their health care advance
directives. The MHA also provides instructions for helping you to understand and complete your advance directives.
We, at Legal Services for the Elderly, have created an interactive version of the form that you can
fill out online and then print.
